Case Description
This is a saga. I had an implant in 2004 and waited six months for it to heal. Then in April, 2005, I had implant cap put on. The dentist did not use a permanent method?? He used some form of screw in. He said that this would be better if it had to be removed. Subsequently, I experienced pain. Then the cap kept coming loose. I went back numerous times, but without success. Finally in November 2005, the dentist put some permanent type of cement, lock tight( I later found out). This lasted until last month. After flossing, I noted that the cap felt loose again, although it had not come off. I went to see him early May and he decided that it would be best to splint it to the adjacent tooth. He claims there is bone loss underneath and the cap will not hold. He put a steel rod between the two teeth. I experienced throbbing pain for a week. Now it has subsided, but the tooth procedure feels uncomfortable. The implant cap was not spliced to another implant, but to a natural tooth with a cap on it. He based this procedure on a clinical update by a periodontist. The implant cap was done by a dental surgeon. The implant cap was done by a prosthodontist. He has already connected the implant to the adjacent tooth. . I went to see him . He looked at the xray and indicated that it looks fine. He claims that the cap came loose because the dentist who did the implant did not make it wide enough. He claims that he has done this procedure many times?? By the way, same surgeon did another implant prior to this and I had no problems. Unfortunately, the previous dentist retired. This one was finished in 04. Subsequently, I went to see the dentist (periodontist) who wrote the clinical update on splinting. He agreed that the implant cap should not have been splinted to a natural tooth and called my prosthodontist to fix the situation. I will see him on Wednesday. The consultant advised that the implant was too narrow. He said that it was also loose. He felt that the size of the implant has caused the implant cap to come loose. My prosthodontist also thought this. He advised a wider abutment or redoing the implant or putting in two implants. The xray shows bone loss beside the implant. Another suggestion was building this up. I have had other opinions on this matter on this site. Some dentists think that the width of the implant should not matter if the implant cap was fitted correctly. Any opinions on this. The implant was put in by a very fine dental surgeon. I find it hard to believe that he did not put in the correct implant size. Does anyone have an opinion on this new matter. I really do not have pain now, but it is not entirely comfortable. I have advised the two dentists to confer, since I am sure the dental surgeon will say that it is the prosthodontists fault & vice versa. My main questions: 1. If the implant is narrow, wouldnt the prosthodontist make the implant cap accordingly? 2. If the implant cap could not be made for the narrow implant what are my options now? I have spent over $2500 on this tooth and am entirely frustrated.
