Case Description
I had a root canal about a year ago on the uppper right last molar( next to wisdom, which was previously extrtacted). Subsequently it was sensitive to heat, not cold. I went back several times. He cleaned it again. Then he recommended an Apicoectomy Based upon my research of this procedure, I was hestitant to proceed. He wanted another $800 to do this in addition to the root canal payment. While the cost was an impediment, he also indicated that the procedure only had a 50% chance of success. I was also concerned about possible injjury to the sinus.

I went back to see him last week. He said that he could not see any infection and perhaps the sensation was caused by a sinus problem. He said the root canal looked fine.

I am sitll having pain when drinking hot liquids. It is also sensitive when I touch the outside of the cap. The cap was put on prior to the root canal. It was restored with pulp and a post.

At his point, I can live with the problem, but am wondering if anyone has a suggestion.

Sounds like you have an incomplete fracture of the tooth with the symptoms that you describe. My personal opinion on this matter is to have the tooth extracted, the site grafted and place an implant in this area and it is more probable you with have a higher success rate as long you have the right restorative dentist that can deal with have the right amount of occlusion for this area. In any case you should not have any temperature sensitivity and it is more probable that you are having a fracture. I do not recommend doing the apicoectomy.

Hi, I know I''''m posting a response here monthes later. If you''''re still in pain, this is definately not normal -- and you should not have to live with it. I definately agree with last response. It sounds like a failed root canal and/or fracture. Apicoectomy has a good chance of not working... the most predictable option would be an extraction and implant. If you need a referral to an implant specialist in your area, please call my office at 561-433-9694. Also, you may want to go to Nova Southeastern Dental School --- they have an excellent Endodontics (root canal speciality) and Periodontics program (implant speciality). I am a Nova graduate and do know people in these programs so if you need assistance getting in there please call my office. Nova is very inexpensive with fees so this would save you a lot of money if finances are a concern.
