Just some clarifiction. The implant cap was splinted to a cap rather than a natural tooth. I do not know if this makes any difference.

That just depends if the cap is on an implant tooth or a natural tooth that had been capped. Like I said implant to implant, okay. Implant to natural tooth or natural tooth with a cap, bad.

Hello, I am assuming that this is Ms. Peggy Cook. I have not received any information from your dentist. If it is Dr. Gregory Tarantola, then I would be very surprised if he is splinting your implant tooth to a natural tooth. As I know he follows the same philosophy of occlusion and reconstruction as I do. In any case, I would be more than happy to troubleshoot your problem as best as I can. I did noticed that you have indicated that the implant tooth is tooth #18. This does clarify some things in regards to screw loosening. One, single posterior implant teeth on the lower jaw especially ones are the very posterior of the mouth are very prone to loosening due to high stresses in this area from occlusion or the bite. Once excessive force is displaced into a posterior implant tooth, energy from that force has to dissipate to either the bone or to a weak area that can move such as the screw. Once enough force is delivered only tooth things can happen. One, deintegration of the implant or loosening the screw. In order to combat these issues, splinting is often the treatment of choice. However, splinting to a natural tooth is not good choice, as it can have detrimental problems to an implant tooth. The only splinting that can be done to an implant tooth would be either to another implant or to design a prosthetic over the natural tooth to allow it to move independently from the implant. I am curious to find out what information I receive from Dr. Tarantola.
